Disruptive Transformations in Health Care:

Advances in medical technology have altered the need for certain types of surgery to be performed in traditional inpatient hospital settings. Less invasive surgical procedures allow a growing number of medical treatments to take place on an outpatient basis. Hospitals face growing competition from ambulatory surgery centers (ASCs) that utilize innovative technology. The competitive threats posed by ASCs are important, given that inpatient surgery has been the cornerstone of hospital services for over a century. Disruptive innovation is heralded as a way to lower health care costs, while improving the quality of and accessibility to care. Yet, regulations that aim to control the effects of disruptive innovation in the hospital industry can limit access and keep costs high. Understanding better how surgical volume shifts between hospitals and ASCs is important to informing health policy debates. The study examines disruptive innovation theory in health care and the effects of policy reforms on the utilization of appendectomy, cholecystectomy, and bariatric surgery.

Health is universally regarded as an important index of human development. Improvements in health would translate into higher incomes, higher economic growth and accelerated declines of poverty. Without understanding the factors which play an important role in health care utilization, efforts to promote demand for health care will not offer a pragmatic solution. Increasing utilization of health services is the prime goal for many developing countries. In this study, potential biases are controlled by an advanced statistical technique. The results of this study can provide unbiased estimates of the magnitude of household health care expenditure in Bangladesh. These findings will call for policy makers to either design new health policy or strengthen existing ones taking into consideration of various factors that are identified in this study in order to improve health care system in developing countries like Bangladesh.

Inter-district Variation in Health Care Services in Balochistan

The existence of regional inequalities in health care services is a common phenomenon at global level. Efforts are made to reduce the regional inequalities by promoting equitable access to primary health care (PHC) services and facilities. This study explores the inter-district variation in primary health care services and development in Balochistan. The study uses a set of twelve (12) PHC related indicators, out of them ten (10) are related to curative health care and two (2) indicators are selected covering the curative aspect of primary health care. By using 0-1 transformation value technique, the composite indexes are calculated for curative and preventive health care services. Districts are ranked as under developed, moderately developed, developed and less developed. There are found sharp inter-district variations in curative as well as preventive health care services in Balochistan. In order to promote the primary health care services in Balochistan the proposed recommendations include ensuring equitable provision of health professionals particularly in terms of female doctors, and decentralization of tertiary care services in order to promote primary health care services.

Demand-Supply Analysis of Health Care in Bangladesh

The study attempts to estimate the factors which influence the demand for health care as well as supply of health care in Bangladesh. A binary logistic regression has been employed with two urban samples in Bangladesh for analyzing the demand for health care. The empirical results of this research show that the estimated coefficient of price variable is negatively related to demand for health care in both the study areas. In both the study areas, there is no remarkable patient having health insurance because the concept of health insurance is not known to them. In demand analysis, findings of the study reached the conclusion the price of health care should be reduced in such a way that the patient gets maximum health care services. Government may set up some guidelines on consultancy fees of the private providers. On the other hand, ordinary least squares method has been employed to estimate the supply of health care in Bangladesh. The empirical results of the supply of health care show that price of physician visit and gross domestic product are positively related to supply of health care and it is statistically significant at 5 percent level.
